Notice that there is a separate bar displayed below the life bar - it is the rage bar and it will fill up as T-Rex takes damage. Notice also the three spiked logs held by two rock pillars around the arena. Stand in front of one and fire the handguns at T-Rex until his rage bar fills up to the top. Once he becomes fully enraged, he will try to ram Lara. Move out of the way by making an "Adrenaline Dodge" to either of the sides as the screen blurs, then shoot him as the red target marker appears. (To easily line Lara up with one of the spiked logs find the areas without any grass [lightly colored dirt]).
